Additional Information:
Attach any files that could be used for your Notable. For faculty/staff Notables, we use portraits in the GGC Directory unless provided with a new one. If the total file size exceeds 5MB, please email the file to as an attachment with the subject "For Notables."
Note that form submissions with file attachments will take longer to submit.
